Patient Forms

No one likes paperwork, we get it. But, if this is your first-time visiting RAO, there are some forms you will need to complete. To streamline the process, we have made most of our forms web-based, so that you can complete them online or print them off to complete at home and bring with you to your appointment. First-time patients should complete all four pages of the New Patient Form (New Patient Form Button below). Please note, if you have seen ANY of our Clinicians at a previous location, but this is your first time seeing us at RAO, you are still considered a new patient and must complete the new patient paperwork.
